A neutral informational resource

This Registry maintains organized provider entries so families and care coordinators can navigate senior care and end-of-life services in Maine in a stable, neutral format. It is not an advertising platform, referral service, or lead-generation program.

Its purpose is to provide families and care coordinators with a clear reference point during periods of care transition — organized by situation, service category, and region.

Participation supports

  • Administrative verification of core provider information
  • Consistent structured presentation of registry entries
  • Ongoing listing maintenance and accuracy
  • Public access by families during care decisions

Participation does not mean

  • Endorsement, ranking, or recommendation
  • Guaranteed referrals or inquiries
  • Preferential placement over other listings
  • Performance evaluation or quality certification
Providers remain responsible for representing their services accurately and for all aspects of service delivery. The Registry is not affiliated with any hospital, government agency, or healthcare system.

How inclusion works

Submit a provider inquiry

Complete the form on this page or email the registry administration contact. Provide core organization information and preferred participation level.

2

Registry administration reviews the submission

Submissions are reviewed for completeness and category fit. Registry administration will contact you by email if clarification is needed.

3

Core information is verified and formatted

Listing details are confirmed and formatted to match registry standards. Provider entries follow a consistent neutral presentation format.

4

Request inclusion or ask a question

Use the form below to request registry inclusion or to ask a question about participation. Registry administration responds by email, typically within 3–5 business days.

Provider Inquiry Form

All fields marked required must be completed. Registry administration will contact you at the email address provided.

This will be used as the basis for your registry listing description, subject to formatting review.

Registry administration handles all inquiries by email. Submission of this form does not guarantee inclusion. All listings are subject to review for completeness and category fit. Annual participation fees are non-refundable. The Registry may remove listings that no longer meet inclusion criteria — including loss of licensure, confirmed closure, or repeated unresolved complaints — without obligation to provide a refund. Participation renews annually and will not auto-renew without explicit confirmation.
