Care providers who submit inquiry forms or purchase participation are subject to the following: Annual participation fees are non-refundable. The Registry may remove or modify listings at any time if a provider ceases operations, loses applicable licensure, or no longer meets inclusion criteria, without obligation to provide a refund. Submission of an inquiry form does not guarantee inclusion. The Registry reserves the right to decline any listing request without explanation.
