Signs that a reassessment may be needed

  • Supervision needs have increased beyond current coverage
  • Physical care tasks are becoming difficult or unsafe to manage
  • Mobility or transfer safety is uncertain
  • Medication management feels unstable or unreliable
  • Behavioral or cognitive changes are increasing
  • The caregiver can no longer sustain the current level of care
